Needed to Register – Gather the following documents prior to appearing for vehicle registration:
- A title that has been signed by the seller, contains odometer information, the purchase price, and any damage disclosures. Since not all titles do not contain all of this information you will need:
- A completed Application for Certificate of Title and/or Registration for a Vehicle (Form 411007)
- A bill of sale
- An Odometer Disclosure (Form 411077)
- A Damage Disclosure (Form Form 411108)
- Photo ID (driver’s license)
- Proof of Auto Insurance
How To Write
Step 1 – Seller’s Information – Enter the following:
- Seller’s Name
Step 2 – Required Vehicle Information:
- Vehicle Identification Number (VIN)
Step 3 – The seller must provide the buyer’s name:
- Enter buyer’s name
Step 4 – Amount of Sale:
- Enter the dollar amount
Step 5 – Date of Sale:
- Enter the date of the sale on the line provided on the form in mm/dd/yyyy format
Step 6 – Seller’s Signature:
- The seller must provide their signature on the “Seller’s Signature” line
Note* – Your County Treasurer may require that you have this document notarized. Therefore the seller should first call the County Treasurer’s Office, to see if notarization is required in the buyer’s county. Do not sign the document until you’re sure you won’t have to have it notarized. If so, sign before the notary only.
